All students that participate in sports need to have their physical, handbook,and medical consent forms on file in the Athletic Office.  If you have not yet had your physical, you may print off the form below.  Also, the handbook and the medical consent forms are available below.  You may choose to print only the last page of the handbook to save paper. Please make sure all forms are completed and signed before turning in to your coach or the Athletic Office.
